Eligibility

Is it right for you?

Clinicians look at your weight, health history, current medications and labs. Filling out the assessment starts a review. It does not mean a prescription.

Often a fit

  • A BMI of 30 or higher, or 27 or higher with a weight-related condition such as high blood pressure, prediabetes or sleep apnea.
  • You have tried lifestyle changes and want a medical plan alongside them.
  • You can commit to a weekly injection and scheduled check-ins.
  • You are not pregnant and are not planning a pregnancy in the near term.

Not a fit, or needs review first

  • You, or a close family member, have a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
  • You have a history of pancreatitis.
  • You are pregnant or planning a pregnancy.
  • You take medications, such as insulin or a sulfonylurea, that your clinician needs to review for low blood sugar risk.
A licensed clinician decides.

Your clinician may recommend this program, a different option, or no medication at all. Eligibility is never automatic.

Medications

Two medications, and what compounded means.

Semaglutide and tirzepatide are both given as a once-weekly injection. Each one has a brand version and a compounded version. The two are not the same.

Your dose starts low and only rises when your body tolerates the current step.

Semaglutide

A GLP-1 receptor agonist. It mimics a gut hormone that signals fullness and slows stomach emptying.

Brand

Wegovy

FDA-labeled for chronic weight management. Made by the manufacturer to a fixed label, with set pen strengths.

Compounded

Compounded semaglutide

Made by a compounding pharmacy. Not FDA-approved. The FDA does not review compounded drugs for safety, effectiveness or quality before they reach patients.

Tirzepatide

A dual GIP and GLP-1 receptor agonist. It targets two gut hormone pathways that influence appetite and blood sugar.

Brand

Zepbound

FDA-labeled for chronic weight management. Made by the manufacturer to a fixed label, with set pen strengths.

Compounded

Compounded tirzepatide

Made by a compounding pharmacy. Not FDA-approved. Strength and consistency depend on the compounding pharmacy, not a brand label.

How dosing titrates (example only)

Doses titrate gradually, and only if you are tolerating the current dose. Your clinician sets your schedule. The steps below are an example of a published brand-label schedule, not a prescription and not a promise of timing.

Example: semaglutide brand-label steps

  1. 0.25 mgWeeks 1 to 4
  2. 0.5 mgWeeks 5 to 8
  3. 1 mgWeeks 9 to 12
  4. 1.7 mgWeeks 13 to 16
  5. 2.4 mgWeek 17 onward, if tolerated

Example: tirzepatide brand-label steps

  1. 2.5 mgWeeks 1 to 4
  2. 5 mgWeeks 5 to 8
  3. 7.5 mgWeeks 9 to 12
  4. 10 mgWeeks 13 to 16, if needed
  5. 12.5 mgWeeks 17 to 20, if needed
  6. 15 mgWeek 21 onward, highest labeled dose

Compounded schedules are set by the prescriber and are not standardized to the brand label. Medication is dispensed by a pharmacy and billed separately from the program fee.

Timeline

Your first 12 weeks.

Every step has a clinician decision behind it. Nothing changes in the plan without your clinician signing off.

  1. Week 0

    Assessment and clinician review

You complete the online assessment. A clinician reviews your history, medications and labs before any prescription is written.

  2. Week 1

    First dose

You start the lowest dose on your schedule, with a written injection guide and a check-in message from your care team.

  3. Weeks 2 to 3

    First dietitian session

Protein targets, meal timing and hydration, planned around the appetite changes you are already noticing.

  4. Week 4

    First check-in

Your clinician reviews how you are tolerating the dose. The next step happens only if you are doing well on the current one.

  5. Week 8

    Dose review and labs

A clinician visit on dose, side effects and goals. Follow-up labs are drawn if your clinician orders them.

  6. Week 12

    Progress review

Your weight trend, labs, habits and next-phase options reviewed together. Your clinician decides what continues.

Side-effect support

What helps in the first weeks.

Most side effects are gastrointestinal and tend to ease as your body adjusts. Tell your care team early rather than pushing through.

Eat smaller meals

Stop when you feel satisfied, and eat slowly. Large meals are the most common trigger for nausea.

Keep fluids up

Steady water intake helps with constipation and dizziness. Spread it across the day.

Go easy on rich food

Fried and very fatty meals can make nausea and reflux worse, especially in the week after a dose step.

Move after meals

A short walk after eating can help with bloating and the heavy, full feeling many people report.

Get emergency care now for:

Severe or persistent stomach pain, especially with vomiting, which can signal pancreatitis. Also yellowing of the skin or eyes, fainting, or signs of dehydration such as very little urine. Message your care team for milder symptoms. Do not stop or change your medication on your own before you speak with your care team.

Important Safety Information

WARNING: RISK OF THYROID C-CELL TUMORS. In rodent studies, semaglutide and tirzepatide caused thyroid C-cell tumors, including medullary thyroid carcinoma (MTC). It is not known whether these medications cause MTC in people. Do not use if you or a family member has had MTC or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Symptoms of thyroid tumors include a lump or swelling in the neck, trouble swallowing, shortness of breath, or a persistent hoarse voice. Tell your clinician right away if you notice any of these.

Common side effects include nausea, diarrhea, vomiting, constipation, stomach pain and indigestion. Serious side effects can include pancreatitis, gallbladder problems, low blood sugar (especially with insulin or a sulfonylurea), and kidney problems, which can follow dehydration from vomiting or diarrhea. Stop the medication and seek care right away for severe abdominal pain that does not go away.

This medication is not for use during pregnancy. Compounded versions are not FDA-approved. This is not a complete list of risks. Talk with your clinician about your full health history and all of your medications. Results vary, and individual results vary.
